The Business Office at Community Hospital of Bremen is here to assist you with the Registration and Billing functions related to your hospital visit.

Patient Registration
We strive to make patient registration as simple and convenient as possible, while making sure the information collected is accurate in order to avoid any billing issues later in the process. We have registrars in multiple areas of the hospital to assist you. It is important that you be prepared with all of your demographic information and your insurance card(s) when you come to register. It is our policy to verify all of your information at each visit and to copy or scan your insurance card(s) each time you are registered. The filing of your insurance claims is based on the information provided at the time of registration. Corrections or updates to information after the time of service are the patient’s responsibility.

Consent For Treatment of Minors
Patients under the age of 18 may consent to their own treatment if the following conditions apply:

  1. They are married
  2. They are legally emancipated
  3. They are at least 14 years of age,not dependent on a parent for support, living apart from their parent(s), and manage their own affairs (all of these must apply).
  4. They are in active military service
  5. They are authorized to consent to their own health care by any other statute

If these conditions do not apply, and it is not an emergency, the patient will need to return with a parent or guardian who can give consent for the minor to receive any elective outpatient services. If a patient is under the age of 18 and requires treatment for a venereal disease, parental consent is not required. If a patient is under the age of 18 and pregnant, parental consent is required, unless the conditions in part 3 of the list above apply. This policy is based on Indiana Code IC16-36-1-3.

  • Billing
    Community Hospital of Bremen’s Billing Department will file insurance claims as a courtesy if we have complete and accurate information. It is the responsibility of the patient to know their insurance coverage policy and to follow-up with their insurance company on any unpaid claims. Any claims that remain unpaid by insurance or any denied claims are ultimately the patient’s responsibility. Remember that an insurance policy is a contract between the patient and their insurance company and the patient has the final responsibility for payment of their hospital bill(s).

    In order to continue our hospital’s healing mission, it is essential that payment be received for the services we provide. Co-payments are due at the time of service. Patients may also be notified before a scheduled appointment of any known deductible amount or co-insurance amount. If you have questions regarding your co-pays, deductible, or co-insurance, please contact your insurance company.

    Knowledgeable staff is available to answer your billing questions Monday-Friday from 7:00 a.m.-5:30p.m. Please call our main switchboard number at 574-546-2211 and the operator will be happy to forward your call to the billing representative who handles your account. To expedite your call, please be prepared to provide the operator with the patient’s name, account number, and date of service.

    Please note that you may receive bills from physicians who practice at Community Hospital of Bremen that you did not see in person. These bills are for professional services such as reading x-rays, administering anesthesia, pathology, or interpreting other tests. If you have questions regarding these bills, please contact their offices directly.

    Payment Options
    For your convenience, Community Hospital of Bremen accepts payments via cash, personal check, money order, or VISA, MasterCard, Discover, and American Express. Payments can be made in person at the Main Information Desk or at the ER Registration Desk. Payments can also be made online by clicking the Online Bill Pay link.     Financing
    Community Hospital of Bremen and 1st Source Bank have joined together to offer a financing option for your hospital care. This option enables you to make a reasonable monthly payment at a competitive fixed rate. You may qualify for this loan regardless of your past credit.     Billing Agencies
    You may also be contacted by an outside billing agency for one or more of your hospital accounts. If you have questions regarding something you have received from one of these agencies, please contact them directly to discuss payment options or billing questions.

    State Sponsored Plans
    If you are currently without health insurance coverage and are interested in finding out about the Healthy Indiana Plan or Hoosier Healthwise, Community Hospital of Bremen is an enrollment site for both programs and we have applications available. Business Office staff members can assist you in filling out these applications, but request that you please make an appointment. Please call 574-546-8048 to inquire about these programs or to make an appointment.

    Financial Assistance
    Community Hospital of Bremen has Financial Assistance available for patients who qualify. Please contact Business Office staff at 574-546-8048 to find out more information. It is the responsibility of the patient to provide the required documentation at the time of the application.
